Our Home- and Community-based Client Experience Power Survey is a concise, two-page survey with 21 questions and a comment section. The survey features large print, making it easier for elderly clients to complete. The survey focuses on the factors that are most important to clients who receive services and support while living at home and/or out in the community. Survey questions are structured around topics such as overall experience, likeliness to recommend, care/services, staff, and demographics. The survey is driven by our exclusive Net Promoter Score® approach.
